POSITION DESCRIPTION – LIAISON SUPPORT ASSISTANT

POSITION SUMMARY

The role of the liaison support assistant is to provide clerical support to the Liaison Team, assist in referral growth and strengthen relationships with internal and external referral sources.

Business Development Manager

Chief Executive Officer

QUALIFICATIONS

•High School education or equivalent

•Two [2] years’ experience in a professional office environment

•Ability to type/data enter information in an efficient and accurate manner

•Thorough knowledge and operation of computers

•Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state

DUTIES AND RESPONSIBILITIES

A.Assists with the coordination of referrals

B.Data enters patient clinical data into the software system

C.Generates and distributes reports from various software systems for Clinical Management

D.Maintains confidentiality of patient and staff information

E.Handles general inquiries, whether received by telephone, mail, or in person

F.Assists liaisons to provide appropriate coverage for partner facilities

G.Communicates with physician offices and other referral sources to increase referral volume

COMMUNICATION

A.Maintains ongoing liaison and communication among the staff

B.Ensures the accuracy of public information materials and activities, representing the agency in professional settings and within the community at large

C.Collaborates with health and health related organizations to increase and improve services to the community

PROFESSIONAL STANDARDS

A.Protects patient privacy and confidentiality by being discreet and professional in sharing

information on a need to know basis

B.Adheres to all policies and maintains a professional standard of conduct, projecting a positive image of the agency

C.Serves as a role model for all staff
