Overview: The CNA must provide care activities for the patient as directed in the patient care plan.
1. Personal Care Tasks:
- Bathing (bed, tub, shower)
- Routine skin care
- Hair care (comb, brush, shampoo)
- Oral hygiene (brush, floss, dentures)
- Dress/assist with dressing/grooming
- Assist with toileting
- Nail care (file, and clean fingernails. File and clean toenails only; DO NOT TRIM)
- Shave
- Prepare meals
- Feed/assist with feeding
- Encourage proper nutrition
- Assist with ADL’s
- Care of incontinent patients
2. Mobility:
- Assist in and out of bed/wheelchair
- Use of Hoyer Lift for transfers
- Assist with ambulation
- Encourage physical activity as condition permits
- Transfer in and out of handicap vehicles
- Turns/repositions patient in bed
- ROM
- Assist with patient ambulating with walker
- Accompanies patient to MD, appointments, etc.
3. Healthcare Activities:
- Temp, pulse, respirations, blood pressure
- Obtain daily weight
- Documents I & O
- Catheter care with soap and water
- Empties bedside bag and records output
- Collects urine, stool, sputum specimens
- Assist with self-administration of medications
4. Observation and Documentation:
- Document all tasks performed on duty sheets
- Observe and report to Supervisor any change in patients condition
- Observe and document food and fluid intake
- Continually make sure of patients comfort and safety
- Report any known abuse or suspected abuse to Supervisor
- Report any unsafe conditions in home to Supervisor
5. Environment:
- Follow care plan for tasks to be performed
- Make bed, change linens as ordered and PRN
- Maintain a clean and safe environment
- Wipe up spills promptly
- Clean equipment as ordered per procedure
- Empty trash at the end of each shift
- Keep side rails up at all times of bed patients
Activities not allowed; unless authorized and trained for proxy care:
- Medication administration-Cannot give any form of medications-oral, topical, inhaled, rectal
- Wound care
- Bowel Program
- Tube feeding
- Catheterizations
- Suctioning
- Blood glucose monitoring
- Ostomy care
Physical Requirements:
- Have adequate transportation to and from job assignments accepted from CareMaster. Necessary to get in/out of vehicle, walking to/from vehicle and up/down stairs as required.
- Requires full range of motions including handling, lifting, turning, and transferring adult patients and in use of equipment necessary to provide care to the patients.
- Bending, squatting, walking, and standing as needed for the duration of the shift to provide necessary nursing services, personal support services, and preventive measures as required.
- Be able to lift 50 pounds.
- Required vision and hearing within normal range.
- Required to work under stressful conditions and irregular hours as dictated by the patients of CareMaster Medical Service.
- Be able to work providing services in accordance with the laws governing CNA/NA/PCA practice in the State in which services will be provided to patients of CareMaster Medical Service.
