Providence is calling a Community Health Worker (Care Coordination, Days, 1.0 FTE) to Providence Medical Group in Polson, Montana. - General Outreach Duties:
- Actively seek opportunities for outreach with a focus on hard to reach populations and our identified “high-need” communities.
- Conduct presentations about available services to local community based organizations.
- Maintain and strengthen relationships with key community stakeholders.
- Work with other caregivers within the Population Health Department and Primary Care Clinics to coordinate outreach events and direct service opportunities; communicate outreach events.
- While performing outreach activities, recognize possible community resources and bring back information to supervisor.
- Maintain knowledge of Providence Population Health programs and provide community with appropriate linkages as requested.
- Act as representative for Providence Health and Services at professional and network meetings, as needed.
- Attend and participate in regular staff and department-wide meetings.
- Enrollment Duties (for CHWs in outreach and enrollment track):
- Provide outreach in the community to increase awareness of Medicaid and SNAP (food stamps).
- Administer screening tool to identify and refer community members/patients who are eligible for Medicaid and SNAP (food stamps).
- Navigation/Linkage to Resources Duties (for CHWs in the Navigation/Linkage track):
- Provide on-site screenings of risk-factors that impact an individual’s well-being in community settings.
- Refer clients/patients to the appropriate programs in the community that meet the needs identified in the screenings.
- Build relationships with clients/patients by asking appropriate questions in a compassionate manner and uses good listening skills.
- Follow up and assist families who are having difficulty accessing resources to which they were referred.
- Administer client satisfaction survey to clients assisted.
- Fully understand and is able to explain the issues for which they are screening and the resources available to meet those needs.
- Work closely with Population Health and Primary Care caregivers to ensure a seamless flow for the patients.
- Documentation/Information Management:
- Document all outreach events and direct services including maintenance of log and sign-in sheets
- Implement and monitor collection of program's evaluation data (e.g. surveys)
- Ensure that program statistics are entered into the appropriate database (e.g. Carescope or other Population Health database) per Department deadlines
- Submit monthly reports to Manager and/or Department Data Specialist.
- Other duties as assigned
- Develop, maintain and demonstrate up-to-date knowledge on Population Health issues by attending seminars, trainings, or conferences as required by the Department, reading materials or other methods.
- Develop, maintain, and demonstrates up-to-date knowledge of hospital and community resources.
- Participate in staff development activities.
- Human Resources:
- Employee completes all tasks and duties accurately and timely within assigned shifts. Demonstrates initiative to perform job duties autonomously.
- Attend Mandatory and other education offerings per department and organizational requirements. May assist with education within the department.
Required qualifications for this position include:
- H.S. Diploma or GED
- Related Community Experience - living, engaging and/or working with hard to reach populations and our identified “high-need” communities
- Valid Driver’s license and auto Insurance
Preferred qualifications for this position include:
- Associate's degree in Health or Social Work related field
- Bachelor's degree in Health or Social Work related field
- Preference will be given to applicants who have experience working with hard to reach populations and our identified "high need" communities
