Job Title: COMMUNITY HEALTH WORKER I Department: COMMUNITY OUTREACH FUNCTION: The Community Health Worker (CHW) will facilitate health promotion, coalition development, and health prevention activities. Works under supervision of the Community Outreach Department Director and the Program Manager of the Center for Healthy Living. MAJOR TASKS AND RESPONSIBILITIES: • Works with other program staff to support health living resources in the community. • May join coalitions or community groups to build or maintain community partnerships and shared resources. • May assist Program Manager in preparing presentations regarding assigned program area for citizens, clients, staff, management, and/or elected officials. • Performs related work as assigned. • Distributes chronic disease self-management information to the community. • Assists Program Manager in implementing interventions for providers and/or the community. • Assists Program Manager in data collection through pre- and post-intervention questionnaires. • Completes community resource referrals for NET Health clients. • Assist clients with eligibility requirements and applications for programs offered at the Center for Healthy Living. • Performs client navigation to internal or external resources for health management. • Maintains client files and documentation as required by programs. • Completes health screenings at the Center for Healthy Living, and at community events. • Participates in outreach events on behalf of the Center for Healthy Living. • Assists health educator with group and/or one-on-one education classes on chronic disease self-management and prevention. • Handles confidential health information with discretion and adheres to HIPAA rules and requirements. KNOWLEDGE, SKILLS, & ABILITIES REQUIRED TO PERFORM IN THIS POSITION: • Must have strong interpersonal and communication skills and can get along with co-workers and other NET Health employees. • Ability to work independently, organize work, plan, and maintain accountability for time and work schedule. • Individuals who are bilingual in Spanish and in English is preferred • Experience in education and outreach helpful. • Must possess a reliable car, Texas driver’s license, and always maintain current insurance. EDUCATION Preferred Certified by the Texas Department of State Health Services as a Community Health Worker, enrolled in a program to become certified, or willing to become certified with one year of employment. Maintenance of CEU hours required with certification. High school diploma or equivalent required. WORKING CONDITIONS: Demonstrates the following positive performance attributes: Observes work hours of 8:00 – 12:00 and 1:00 – 5:00 unless other arrangements are made with supervisor. Is willing to occasionally work different hours (including evenings and weekends) as workload requires. Must be able to spend up to eight (8) hours walking, standing, and/or sitting. Must be able to lift up to 25 pounds.
