As an Acute Care Case Manager, you will play a critical role in continuing the healing mission of Deaconess.

Job Overview:

Day to day, you would be assessing the needs of the patient and family to coordinate, monitor, evaluate, and advocate for multiple services to meet the specific patient's complex needs. You would collaborate with a multidisciplinary team regarding the patient needs based on the assessment.

Required: Certifications/Licenses in IN or compact licensure states:

• Active Registered Nurse (RN)
